Rituximab for Pediatric Central Nervous System Inflammatory Disorders in Alberta, Canada

Background:
Early and effective treatment of central nervous system (CNS) inflammatory disorders is vital to reduce neurologic morbidity and improve long-term outcomes in affected children. Rituximab is a B-cell-depleting monoclonal antibody whose off-label use for these disorders is funded in the province of Alberta, Canada, by the Short-Term Exceptional Drug Therapy (STEDT) program. This study describes the use of rituximab for pediatric CNS inflammatory disorders in Alberta.
Methods:Rituximab applications for CNS inflammatory indications in patients <18 years of age were identified from the STEDT database between January 1, 2012, and December 31, 2019. Patient information was linked to other provincial datasets including the Discharge Abstract Database, Pharmaceutical Information Network, and Provincial Laboratory data. Analysis was descriptive.
Results:Fifty-one unique rituximab applications were identified, of which 50 were approved. New applications increased from one in 2012 to a high of 12 in 2018. The most common indication was autoimmune encephalitis without a specified antibody (n = 16, 31%). Most children were approved for a two-dose (n = 33, 66%) or four-dose (n = 16, 32%) induction regimen. Physician-reported outcomes were available for 24 patients, of whom 14 (58%) were felt to have fully met outcome targets.
Conclusion:The use of rituximab for pediatric CNS inflammatory disorders has increased, particularly for the indication of autoimmune encephalitis. This study identified significant heterogeneity in dosing practices and laboratory monitoring. Standardized protocols for the use of rituximab in these disorders and more robust outcome reporting will help better define the safety and efficacy of rituximab in this population
Short-term variations of surface magnetism and prominences of the young sun-like star V530 Per

V530 Per is a solar-like member of the young open cluster α Persei, with an ultra-short rotation period (P∼0.32d). We report on two spectropolarimetric campaigns using ESPaDOnS, aimed at characterizing the short-term variability of its magnetic activity and large-scale magnetic field. We used time-resolved spectropolarimetric observations obtained in 2006 and 2018 and reconstructed the brightness distribution and large-scale magnetic field geometry of V530 Per through Zeeman-Doppler imaging. Using the same data sets, we also mapped the spatial distribution of prominences through tomography of Hα emission. We reconstruct, at both epochs, a large, dark spot occupying the polar region of V530 Per while smaller (dark and bright) spots were reconstructed at lower latitudes. The maximal field strength reached ∼1 kG. The prominence pattern displayed a stable component that was confined close to the corotation radius. In 2018, we also observed rapidly evolving Hα emitting structures, over timescales ranging from minutes to days. The fast Hα evolution was not linked to any detected photospheric changes in the spot or magnetic coverage.

In finite group theory, chief factors play an important and well-understood role in the structure theory. We here develop a theory of chief factors for Polish groups. In the development of this theory, we prove a version of the Schreier refinement theorem. We also prove a trichotomy for the structure of topologically characteristically simple Polish groups.
The development of the theory of chief factors requires two independently interesting lines of study. First we consider injective, continuous homomorphisms with dense normal image. We show such maps admit a canonical factorisation via a semidirect product, and as a consequence, these maps preserve topological simplicity up to abelian error. We then define two generalisations of direct products and use these to isolate a notion of semisimplicity for Polish groups.

Young solar analogs reaching the main sequence experience very strong magnetic activity, directly linked to their angular momentum loss through wind and mass ejections. We investigate here the surface and chromospheric activity of the ultra-rapid rotator AP 149 in the young open cluster alpha Persei. With a time-series of spectropolarimetric observations gathered over two nights with ESPaDOnS, we are able to reconstruct the surface distribution of brightness and magnetic field using the Zeeman-Doppler-Imaging (ZDI) method. Using the same data set, we also map the spatial distribution of prominences through tomography of H-alpha emission. We find that AP 149 shows a strong cool spot and magnetic field closed to the polar cap. This star is the first example of a solar-type star to have its magnetic field and prominences mapped together, which will help to explore the respective role of wind and prominences in the angular momentum evolution of the most active stars.
Quality Assessment of Reporting of Economic Evaluation in Cardiac Sugery: Has it Improved?

Objectives
Cardiac surgery has seen substantial scientific progress over recent decades. Health economic evaluations have become important tools for decision makers to prioritize scarce health resources. The present study aimed to identify and critically appraise the reporting quality of health economic evaluations conducted in the field of cardiac surgery.
MethodsA literature search was performed to identify health economic evaluations in cardiac surgery. The consolidated Health Economic Evaluation Reporting Standards (CHEERS) statement was used to assess the quality of reporting of studies.
ResultsA total 4,705 articles published between 1981 and 2016 were identified; sixty-nine studies fulfilled the inclusion criteria. There was a trend toward a greater number of publications and reporting quality over time. Six (8.7 percent) studies were conducted between 1981 and 1990, nine (13 percent) between 1991 and 2000, twenty-four (34.8 percent) between 2001 and 2010, and thirty (43.5 percent) after 2011. The mean CHEERS score of all articles was 16.7/24; for those published between 1980 and 1990 the mean (SD) score was 10.2 (±1.4), for those published between 1991 and 2000 it was 11.2 (±2.4), between 2001 and 2010 it was 15.3 (±4.8), and after 2011 it was 19.9 (±2.9). The quality of reporting was still insufficient for several studies after 2000, especially concerning items “characterizing heterogeneity,” “assumptions,” and “choice of model.”
ConclusionsThe present study suggests that, even if the quantity and the quality of health economics evaluation in cardiac surgery has increased, there remains a need for improvement in several reporting criteria to ensure greater transparency.
The Evolution of Surface Magnetic Fields in Young Solar-type Stars

Surface rotation rates of young solar-type stars display drastic changes at the end of the pre-main sequence through the early main sequence. This may trigger corresponding changes in the magnetic dynamos operating in these stars, which ought to be observable in their surface magnetic fields. We present here the first results of an observational effort aimed at characterizing the evolution of stellar magnetic fields through this critical phase. We observed stars from open clusters and associations, which range from 20 to 600 Myr, and used Zeeman Doppler Imaging to characterize their complex magnetic fields. We find a clear trend towards weaker magnetic fields for older ages, as well as a tight correlation between magnetic field strength and Rossby number over this age range. Comparing to results for younger T Tauri stars, we observe a very significant change in magnetic strength and geometry, as the radiative core develops during the late pre-main sequence.

Cost of home and hospital care for patients with cystic fibrosis followed up in two reference medical centers in France

Objectives: In France, new guidelines for clinical practices concerning cystic fibrosis came out in 2002, underscoring the need for early and intensive management of this disease. Because no recent health economic studies on cystic fibrosis in France were available, we conducted a cost-analysis study before the new guidelines were put into practice, with a view to a later study on the medical and economic impact of these guidelines.
Methods: A cost-analysis study was performed of the inpatient and outpatient costs of patients with cystic fibrosis for the 2000–2001 period. The various direct costs were estimated on a sample of sixty-five adult and pediatric patients managed for cystic fibrosis in two reference medical centers. Data were obtained from medical records, and questionnaires were filled out by the patients. Analysis was made from the perspective of the French healthcare system.
Results: We studied sixty-five patients, 54 percent male patients and 72 percent children under 18 years of age. The total cost of cystic fibrosis care totaled 16,189 euros per year and per patient. Outpatient costs accounted for 88 percent of the total cost versus 12 percent for inpatient costs; medication costs were the highest with 21 percent of the total cost for home intravenous antibiotic treatments and 49 percent of the total cost for chronic medications.
Conclusions: The results show that outpatient costs were higher than inpatient costs, which could be related to the importance granted to home health care in France, notably for intravenous antibiotic treatments given for pulmonary complications.
UNJUSTIFIED VARIATIONS IN PATIENT MANAGEMENT FOR ACUTE MYOCARDIAL INFARCTION IN THE RHO[ring]NE-ALPES REGION OF FRANCE

Objectives: To examine determinants of use of cardiac procedures after acute myocardial infarction and identify variation factors.
Methods: Observational prospective cohort study of 2,519 patients in 48 centers with a two-level logistic-regression analysis.
Results: Elderly patients were less likely to undergo pre- and inhospital thrombolysis (odds ratios, 0.71 and 0.64; 95% CI, 0.62–0.81 and 0.58–0.69, respectively). The elderly, females, and patients with heart failure on admission were less likely to undergo noninvasive tests (0.74, 0.62, and 0.51; 95% CI, 0.67–0.81, 0.46–0.83, and 0.38–0.68, respectively) and coronary angiography (0.38, 0.53, and 0.67; 95% CI, 0.34–0.42, 0.38–0.74, and 0.52–0.86, respectively) but not revascularization. Hospital factors were more difficult to interpret.
Conclusions: Elderly, women, and heart failure patients underwent fewer cardiac procedures than lower-risk patients. Physicians should change their attitude toward these groups and use advanced procedures, bearing in mind the patients' needs rather than good procedural outcomes.
Influence of antibiotics and food intake on liver glutathione and cytochrome P-450 in septic rats

Experimental sepsis in rats induces a restriction in spontaneous food intake and a drop in liver glutathione, cytochrome P-450 (P-450) and aminopyrine demethylase (AD) activity. The present study was designed to assess the effects of antibiotics alone or when combined with food deprivation on these variables. Eighty-nine male Sprague-Dawley rats were assigned to six groups: control (C), acute infection (experimental pyelonephritis, I), acute infection with antibiotics and food given ad lib. (IA), control with antibiotics (CA), acute infection with antibiotics pair-fed to I (IAR), and sham-operated pair-fed to I (SR). Liver glutathione, P-450 and AD activities were reduced by 45·2, 79·8 and 41·2% respectively in group I. Glutathione and AD significantly increased only in those infected rats given antibiotics and allowed free access to food. P-450 did not normalize within the study period in infected rats receiving antibiotics and food repletion. The risk of drug hepatotoxicity in acute septic states is therefore closely related to the nutritional status. From this point of view, nutritional support is almost as important as treatment of infection.
